Application Process
Click Here to apply for an individual financial assistance grant from the CHN Foundation. If you would like an application mailed to your home, contact the CHN Foundation office at 920-361-6524.

Click Here to apply for a non-profit community group financial assistance grant from the CHN Foundation. Non-Profit Community Groups are permitted to apply for assistance no more than twice per year, and must be located in one of the CHN Service Zip Codes. If you have questions would like an application mailed to your business, contact the CHN Foundation office at 920-361-6524.
After you have completed and returned your application, the CHN Foundation will review and evaluate your needs, and will contact you with regards to their decision.
