 Diabetes Education
Do you or a loved one have diabetes? Are your blood sugars where they should be? Are you unsure how to successfully manage your diabetes? If you have concerns with any of these areas, you may want to read more about the programs that CHN now has to offer. Allow our team to work with you and your provider to get you back on track or help improve your understanding of the disease.
Programs Offered:
Diabetes Self Management Training (DSMT):
DSMT is comprehensive diabetes education taught by a healthcare team which may include a Diabetes Educator, Dietitian, and Pharmacist.
These classes are appropriate for adult diabetics. Whether you are a newly diagnosed patient, or a person who has had diabetes for years, and currently finds yourself struggling to stay controlled, these classes may be for you. You may also be a candidate for this program if you have had some kind of change in your treatment plan, like a change to insulin or other medication.
In these classes, you will learn about the disease and successful self management strategies like; glucose monitoring, carbohydrate counting, improved physical activity and much more. Our staff will give you the support and education you need to be successful in your endeavor to control your diabetes
This program does require a Physician/Provider Referral. If you feel you are an appropriate candidate, and are motivated to change, talk with your healthcare provider about whether you may be a candidate. Medicare and most insurances cover these services.
Medical Nutrition Therapy (MNT):
Medical Nutrition Therapy is also a physician referred service. This service allows a CHN Dietitian to meet with you on a one to one basis to review your diet and nutrition, and provide instruction to improve your diabetes management. Frequently this service is ordered by the physician in addition to DSMT. These two services together help provide the patient with the most complete and thorough training. This service is also covered by Medicare and most insurances.*
If your insurance does not cover these services and you are unable to afford the program cost, you may qualify for Community Care. If so, this assistance program may help pay for part or all of the service. Diabetes Support Group:
CHN Diabetes Education Dept holds a monthly support group on the second Thursday and Friday of each month at different times to accommodate varied schedules. Topics have included, “Diabetes Basics” and “Stocking your cupboards for Diabetes” for example. Upcoming meetings may include topics such as the role of exercise in diabetes, cardiovascular health in diabetes, and carbohydrate counting. Guest speakers and group discussions are the primary formats for this education. It is also a great way to meet other people dealing with a common condition. It allows people to vent frustrations, and share successes they have had in dealing with this disease.

Your Diabetes Educators Are…………
Nancy Gimenez, R.N. Diabetic Coordinator.
Nancy is a native to Berlin. She attained her Bachelor of Science in Nursing in 1995 from UW-Oshkosh, and has practiced in the hospital, homecare and management fields since that time. She has worked at CHN since 2001. She enjoys the patient contact that comes along with diabetes education. “ It really is exciting to watch a patient with very little understanding of diabetes and poor blood sugar control, become knowledgeable about the disease and be able to control it more successfully” , say Nancy. Danielle Krueger, R.D. (Registered Dietitian
Danielle has a Bachelor of Science degree from Viterbo College in LaCrosse, WI. She also has varied experience including; long term care dietitian, and Director of Nutrition Services at Mayville Nursing and Rehabilitative Center. She has worked at Berlin Hospital since 2002 as a Clinical Dietitian. Danielle was very involved in the development of this program. You can reach Danielle at (920)-361-5509.
Sarah Gerber
Sarah received her Bachelor of Science Degree in Pharmacy from UW-Madison in 1997, and just recently earned her Pharmacy Doctorate this May 2007. She has been working at Berlin Hospital since April 2002 as a pharmacist, and was instrumental in developing our diabetes programs. CHN Diabetes Education website
Central Wisconsin is High-Risk for Diabetes. The overall prevalence of diabetes in Wisconsin and specifically the Central Wisconsin area (Green Lake, Adams, Waushara and Marquette Counties) has increased dramatically, as much as 27%. To address the specific needs of the CHN community, the Diabetes Education department has launched a website focused on the Central Wisconsin area.

In December 2006, Berlin Hospital’s Diabetes Self Management Training Program was a proud recipient of a $50,000 grant made possible by the Wisconsin Office of Rural Health. This grant, in part, allowed us to create a comprehensive diabetes education program for people in the Berlin and surrounding areas. We remain very grateful for this opportunity!
