Financial Assistance

Community Health Network (CHN) recognizes a obligation to the community it serves to make available quality healthcare services to anyone, regardless of their ability to pay. We offer a variety of programs to assist those in our community with their financial needs.

Catastrophic Financial Assistance
Catastrophic assistance is available to all uninsured patients who have a balance owed for medical care to Community Health Network in excess of a percentage of net worth. Net worth is based on income and assets.

Caring Hearts
CHN’s Caring Hearts Program provides financial assistance for those who cannot afford to pay. If you received necessary medical care and show financial need, but do not qualify for other financial programs, you may qualify for financial assistance through our Caring Hearts Program.

Caring Hearts will be approved in a four tier program:

  • 100% for applicants with family income less than 200% of the Federal poverty level.
  • 75% for applicants with family income less than 250% of the Federal poverty level.
  • 50% for applicants with family income less than 300% of the Federal poverty level.
  • 25% for applicants with family income less than 350% of the Federal poverty level.

Eligibility is based on a number of factors; including an applicant’s income, assets and family size. Applications are available at all CHN Clinics and at the CHN Business Office, located at Berlin Memorial Hospital or on-line by clicking [Here]. You may also inquire by calling 920-361-5590.
