Personal Payment
Account balances not covered by insurance are to be paid in full within 30 days of billing. As an added convenience to you, we accept Visa, Mastercard, Discover Card and American Express. If your insurance requires a co-pay for emergency room or physician office visits, payment is expected at the time of service. Your payment can be made at the doctor’s office, or at the CHN Business Services Office, located at Berlin Memorial Hospital during regular office hours, Monday through Friday from 7:30 a.m. until 5:00 p.m. On-line payments can be made securely and quickly using our Online Payment System (click).

If you anticipate difficulty paying your account in full within 30 days, a payment plan must be established through the CHN Business Services Office. Please call 920-361-5590. Or you may secure financing through the financial institution of your choice.

Community Health Network (CHN) has an obligation to the community it serves to provide quality health care services to anyone, regardless of their ability to pay. We offer a variety of programs to assist those in our community with their financial needs.

If you do not have any insurance or 3rd party coverage for services that are provided to you or your family at any of the CHN locations, you are eligible for an uninsured discount.

A 30% discount for the uninsured will be applied to inpatient, outpatient, and CHN clinic service charges (excluding cosmetic or other non-covered procedures).

Please bear in mind that amounts applied towards coinsurance, co-payments and your deductible are not eligible for the uninsured discounts.
