Clinical Coordinators

CHN adheres to the Fox Valley Health Care Alliance for all undergraduate and graduate clinical placements; we use this website to ensure continuity with all clinical placements for students within the surrounding area. Below you will find the link to the FVHCA website. Please follow the FVHCA’s instructions for requesting placement and what is required following acceptance. http://www.fvhca.org/StudentClinicalPlacements.html

Following acceptance of a request please direct your students to http://www.communityhealthnetwork.org/site/index.php/student-education/student-forms, here the student(s) will be directed to complete CHN’s forms for placement as well as the needed FVHCA forms. Following the student’s completion of the material they will return those forms to you.

Please complete the FVHCA (Undergraduate/Graduate) Student Faculty Information form which summarizes necessary requirements etc. Please note that all other documentation regarding the student’s health records, immunizations, background checks, etc. will stay housed at the school or university. Should we need additional documentation regarding a student we will request it.
