Students
CHN adheres to the Fox Valley Health Care Alliance for all clinical placements; we use this format to ensure continuity with all clinical placements for students within the surrounding area. As a student you will need to complete two modules on the FVHCA website and fill out two acknowledgement forms of their completion. You will find the modules under Step 2 and the forms under Step 4 in the link below. CHN also requires that you complete an online orientation to our organization, those materials are also below.

Once you have completed the above, please return the following forms to your school’s or university’s Clinical Coordinator:
- FVHCA Form A
- FVHCA Form B
- Student Orientation Outline
- Expectations of Professionalism
- HIPAA Security Quiz
- Intranet Acceptable Use Policy
Please note that if the above forms are not completed and returned to your Clinical Coordinator prior to your clinical you will not be able to attend clinical until CHN receives all required forms.
