CHN's Home Care supports and cares for people who have a chronic illness or a temporary or permanent disability. CHN's Home Care Department provides many support options to help these people with their health or personal needs such as: dressing, cooking and personal hygiene to in-home medical services, therapy services, and Lifeline. Our care options can be provided in the home, at one of our local facilities, or in assisted living situation. Our care plans can be tailored to anyone with nearly any type of need.
Perhaps you've just been discharged from the hospital, are homebound due to illness or injury, or find personal hygiene difficult or even impossible to complete on your own...or is a family member in need of daily living assistance or medical monitoring?
Regardless of the situation, the friendly professionals of CHN Home Care will assess the situation and determine the services most appropriate to meet your needs. We will provide them in the comfort of your home under the direction of your personal physician.
