Healthcare was created because of community need -- and Community Health Network was created for the same reason. Partnerships among providers and the support of area residents who give their time, dollars and talents allow CHN to fulfill our mission of improving the health and quality of life in the communities we serve.

Beyond hands-on medical care our CHN Foundation, a 501 (c) (3) charitable organization, is available to help area children, adults, and families through our Lucille Schultz Caring for Kids, Sandy Tincher Trust Fund and the Ann Marie Frost Arb Scholarship.

Lucille Schultz Caring For Kids, supported by funds raised through the annual CHN Charity Golf Classic, helps children and their families who experiencing catastrophic medical needs. The Sandy Tincher Trust Fund, established in memory of local resident Sandy Tincher, provides financial assistance and supportive services to adults experiencing serious medical problems. The Ann Marie Frost Arb Scholarship, established in memory of local resident Ann Marie Frost Arb, helps a graduating Berlin High School student pursuing a nursing or veterinary degree.

Application Process

After you have completed and returned your application, the CHN Foundation Grants Committee will review your request and contact you with their decision within 4-8 weeks.

The CHN Foundation relies on the generosity of area residents who provide the financial resources to reach out to their neighbors with dire health needs.
