Community Health Network continues a tradition of excellence that began in 1911 when the Berlin Hospital Association was formed.
Today, Community Health Network is a community-based healthcare system which includes Berlin Memorial Hospital; Juliette Manor; CHN Home Care; CHN Pharmacy; CHN Rehabilitation; and the CHN Medical Group, which includes 20 primary and specialty care clinics in the surrounding communities. With more than 750 employees and a partnership with Wild Rose Community Memorial Hospital, CHN serves families in the Green Lake, Fond du Lac, Marquette, Waushara and Winnebago counties.
